Health Insurance
As a service to our individual artist members who work at least 20 hours a week in the arts, the Arts & Cultural Council offers access to several Excellus BlueCross BlueShield health insurance plans listed below at employer rates. All subscribers must keep their artist membership dues current, complete an artist directory form, and reside in the county of Monroe, Livingston, Yates, Ontario, Seneca, or Wayne. New applicants must prove they are a business or self-employed to qualify for group rates, and are subject to approval by Excellus. New artist members should select and join a plan within 30 days of joining the Council; otherwise health plans are only available during open enrollment effective January 1. The deadline for changes is mid December.
Prices below are for quarterly payments with due dates of Dec. 15, March 15, June 15, Sept. 15; and include a $6.00/month* administrative service fee for self-employed individuals for the Council. Late fees are $20, and reinstatement fees due to lateness are an additional $25 if reinstatement is possible. Terminations for nonpayment may result in loss of insurance for one year. Bank charges and late fees for insufficient funds also apply.
Each plan requires application and first payment 30 days prior to the effective date on the 1st of any month.

* The administrative service fee is $8/month for individuals qualifying as an employee of another member organization so the above prices would be $6.00 higher under this circumstance.
